Dr Malouf’s practice collects information that includes, but is not limited to; the patient’s name, date of birth, address, email, phone number, Medicare and Private Health Fund details, and medical history.

• To identify your medical record and provide a quality and accurate health service

• For administrative purposes in running a specialist medical practice: including pre-operative and post-operative calls and hospital interaction for booking surgical services

• For billing purposes, including compliance with Medicare and Health Insurance Commission requirements

• To disclose relevant information to others involved in your medical care, including treating doctors, specialists, and hospital booking staff outside this practice. This may occur through referral to other doctors, surgery at hospitals, for medical tests and in the reports or results returned to us following the referrals
